I understand that if the information contained herein is found to be false, my application can be refused, my volunteer status terminated, and/or said dog fostered or adopted shall be relinquished to Minnesota Boxer Rescue and its agents without refund. I agree/understand that I'm required to monitor my emails, phone calls, voice mails and texts sent (left) by MNBR's volunteers and must respond to MNBR's volunteers within 24-48 hours in regards to the rescue dog (dogs) that is (are) in my possession. I understand that if I do not cooperate with MNBR that the rescue dog (dogs) can and will be taken out of my possession at any time deem to MNBR's discretion.

FOR FOSTERS ONLY: I have been told of the animal's known temperament and behavior and understand a pet is an individual animal with its own mind; there are inherent risks with the guardianship or fostering of any animal and there is no expressed or implied temperament guarantee. As a Foster, I agree to be fully responsible for the pet while fostering the animal until it is safely returned to MNBR or adopted. If any unmanageable conditions or issues arise or are discovered, I shall immediately notify and safely return the animal to MNBR.

I understand and agree to all of the above and I promise and agree to be solely responsible for this animal as of the date listed on this agreement or until the animal is safely returned to MNBR or adopted from MNBR. I agree, indemnify and hold harmless Minnesota Boxer Rescue, or any other individual (representative, rescuer, etc.) from any and all claims of liability for the conduct of this animal on and after the date of this foster agreement. This Release of Liability and Indemnification shall apply to all known, unknown and unanticipated damages resulting from my choosing to foster and control the animal. I also, understand that as a foster parent for the Minnesota Boxer Rescue, I am responsible for cost of all expenses (food, mileage, treats, toys, etc) of the boxer except pre-approved or emergency vet expenses while the foster is in my care.*

The next step to this process is reviewing and agreeing to the MNBR Release of Liability and Waiver. RELEASE OF LIABILITY AND WAIVER • I understand that because I may handle and/or come in contact with animals, it is important to discuss being vaccinated against tetanus with my physician. I release Minnesota Boxer Rescue, from all responsibility that may occur because of my not pursuing this matter further, and I understand whatever decision I make is at my own risk. I have read, understand and agree to the above tetanus information. • I acknowledge and understand that as a volunteer of Minnesota Boxer Rescue, I am not covered by workers’ compensation or any other insurance policy through Minnesota Boxer Rescue, for any damages or injuries I may sustain during volunteer activities. I understand that I am responsible for obtaining health insurance coverage through an independent health insurance company. • I fully understand that as a part of my volunteer work for Minnesota Boxer Rescue, I will come into contact with animals either by directly handling them, fostering or through assisting in their care and adoption. Further, I understand that working with animals carries a risk of injury, and that it is possible that I may be bitten, scratched, and/or otherwise injured • I fully understand that as a volunteer and/or foster home for Minnesota Boxer Rescue, my family may come in contact with animals at Minnesota Boxer Rescue events, and I and my family and/or guests may come into contact with animals in my home if I am fostering an animal. I understand that working with animals carries a risk of injury, and it is possible that my family and/or guests may be bitten, scratched and/or otherwise injured. • My typed named is considered my signature to this volunteer liability release and attests to my intent to hold harmless and release from all liability Minnesota Boxer Rescue or any of its past, present or future officers, agents, volunteers, employees or assigns, from all acts, which are related to the normal performance of required and implied duties. • The terms of this waiver shall continue and remain in effect from the signature date and as long as I volunteer or represent Minnesota Boxer Rescue in any capacity. *
